13 weeks after a facelift, a lady is still swollen in front of her ears and across cheeks, which is making it difficult for her to chew. She just finished a 5-day cycle of the inflammatory Medrol, but it did little to reduce her swelling. She asks if it is a swollen lymph gland, hematoma, or the ridge from the sutures causing the swelling.
In this video, New York Oculofacial plastic surgeon Amiya Prasad, M.D. reviews this lady's pictures, and lists the possible causes of swelling, and how he avoids an extended healing phase in his practice:
1:55 - Samples of facelifts and neck lifts performed by Dr. Prasad
2:10 - The unique insight of the original surgeon, and why their guidance is valuable
2:44 - Why extent of swelling is directly associated with extent of surgery
2:58 - How swelling was extended during Dr. Prasad’s training years, with patients staying overnight, needing multiple drains after surgery, and swelling lasting over a month
3:20 - How Dr. Prasad now performs the same type of facelift surgery, but with less swelling and faster recovery time
3:44 - Natural skin thickness, reddish skin, or a history of rosacea all contribute to longer swelling
4:35 - How hematoma and seroma contributes to extended swelling, even if these may not be the causes
5:16 - Prolonged swelling due to absorption of fluid during healing
6:00 - Why swelling after 13 weeks may not be unusual, depending on the surgeon
6:30 - Possibility of trauma of the parotid gland producing saliva that collects and causes swelling
6:57 - Why asking specific questions to the surgeon who knows your anatomy can help determine the cause of extended swelling
