This video presents steps of the Kuhnt-Szymanowski(KZ) procedure for managing severe involutional ectropion
Case Illustration:
A male patient, 75 years old came to our outpatient clinic with recurrent epiphora and droopy of both lower eyelids for at least 1 year. There was no complaint of blurry vision or red eye. Ophthalmologic examination showed good visual acuity and no intraocular abnormalities. There was lower eyelid ectropion in both eyes. The distraction test was 10 mm on the right eye and 15 mm on the left eye. Snapback examination showed that both lower eyelids need more than 3 seconds before returning to their position. There was a lateral canthal laxity in both eyelids, with more than 2 mm displacement, and medial canthal laxity as shown with 10 mm displacement at right lower eyelid and 15 mm at left lower eyelid.
The patient was diagnosed with involutional ectropion and planned to undergo the Kuhnt-Szymanowski(KZ) procedure.
Principally, in the KZ procedure, the excess skin is excised as a lateral triangle from a blepharoplasty flap, and the lid is shortened under the flap. Postoperatively, the lower eyelid was successfully corrected.
The eye laxity was normal and the patient did not have any complaints during the follow-up.
