What if Itachi got the Eternal Mangekyo Sharingan? During Sasuke and Itachi’s final fight we learned when a Uchicha feels great emotion an unlikely phenomenon occurs in the brain which causes them to activate a Dojutsu known as the Sharingan.
But an even rare exception of Sharingan users will achieve the Mangekyō Sharingan when pushed to the emotional edge. The Mangekyō grants powerful abilities that differ between each eye from person to person. Itachi had gained the Amaterasu in one eye and Tsukuyomi in another. Once a user awakens the Mangekyō in both eyes, they are able to perform Susanoo. However as powerful as Itachi now was he suffered from a disease and as with any high-level dōjutsu, Mangekyo Sharingan has steeper chakra demands. It was causing Itachi to slowly lose his light which means that he almost went blind from overusing the Mangekyo Sharingan.
