Advance Directive forms (also referred to as living wills and medical powers of attorney) give individuals the ability to outline their end-of-life wishes and direct physicians on how to care for patients, especially in the case of resuscitation.

A living will include selecting a health care agent that acts as a Medical Power of Attorney, which grants an agent the ability to make decisions for an individual in case of incapacitation.
