For intumescent white cataracts, it can be helpful to decompress the capsular bag using a needle and aspiration/suction from a syringe. We have featured this technique before and even showed you an automated way of performing this. This technique allows us to lower the intra-lenticular pressure so that we can minimize the risk of capsular run-out in the classic Argentinian Flag Sign formation. When doing this technique, I tend to prefer a bevel-down approach so that the liquefied lens cortex can be more easily aspirated without grasping the capsular bag.
