Defeating all the bosses without taking damage in Venture to the Vile and finding all three endings. When a new plague mutates everything into monsters you get to use it for cool metroidvania upgrades as you rough up everyone to ensure only one eldritch abomination can stay in town.
This one is a bit curious as there are a number of secrets in the game but you can speed through to the first ending quite quickly and easily, missing all of that (I ended with 38% completion) so a lot of the time spent playing will be if you want to look for secrets in the endgame. Similarly, there are some nice mutations but a lot of this is RNG as to whether you find the right materials, either by getting lucky with drops or farming the right creature (and while there's many things to upgrade choosing to boost your claw's damage helps out the most).
I'm not sure as to how many notes you need for ending 3 - I reached the cave missing Ella's 2nd note but got the ending nonetheless.
