"If you're in custody -- which simply means not free to leave the scene for purposes of Miranda -- then you do have the right to have an attorney present during questioning. However that does not mean the officer can't note observations as to how your eyes might have looked or how your breath smelled. It only relates to testimonial evidence; or in other words, things which you say which have been asked by the officer.
For example, if in fact you're not free to leave the scene and the officer says "well gee, have you been drinking?" you can decline to answer that question -- asserting that you would like to have an attorney present during questioning.
