Problem: A 2.5 cm flat lesion was resected en-bloc 6 months prior to this exam.
Patient: Heavy smoker.
Findings: Well healed scar; examination of the top and bottom end of the scar and also the front and back portion of the scar revealed surface mucosa with round pit pattern suggestive of no residual polyp or recurrence. Examine with white light and with NBI. A cap fitted endoscope allows to examine the proximal end of the scar over a fold better. Multiple photos are taken to document the findings from very close up as well to document the pit pattern. Multiple biopsies are taken to confirm normal tissue without any polyp residue or recurrence. A small flat lesion adjacent to the original flat lesion that was noticed before but not removed during the first session was removed this time.
Recommendations: Counsel about stopping cigarette smoking and recommend weight reduction and daily exercise, because cigarette smoking and obesity are risk factors for colorectal cancer.
