"Tabula Rasa" from the album Lamashtu by German progressive death metal band Metamorph. All rights belong to the band. Released in Münster in 2009.

Images are from the pdf booklet from the now defunct metamorph-metal.de, where the album was available as a free download. I re-uploaded the entire zip (exactly as I got it from the site in 2010) here:

As far as I know there is currently no other way to get the album, unless you downloaded it or bought the physical version back in the day.
