A copper T IUCD is typically seen as a T-shaped structure in the uterine cavity on imaging. However, when the vertical bar is positioned horizontally, it indicates malposition. On ultrasound, the echogenic stem (normally vertical) is seen horizontally within the uterus, with the arms of the T deviating from their usual alignment. Transvaginal ultrasound provides detailed visualization, confirming the malposition and identifying possible embedding in the myometrium. On X-ray, the copper T appears as a radiopaque T-shaped structure, with the vertical bar oriented horizontally, clearly showing a rotated position.
This malposition can lead to complications such as contraceptive failure, pain, abnormal bleeding, infection, or uterine perforation. Clinical evaluation and removal or repositioning are often required. In some cases, hysteroscopy may be necessary if the device is embedded. Proper follow-up is essential to prevent further complications.
