I see it every so often ... a cross threaded lug where the tire rotation guy (who must have been new or not trained) did not hand screw the lug nuts before using his impact driver. So much powered torque was needed to remove the lug nut that the lug/stud sheared off into the lug nut...or the threads on the lug/stud was so badly cross threaded that it needed replacement with the lug nut. This video will give you a general idea on how to remove the old lug/stud and replace it with a new one. The video was performed on the front end of a 2004 Honda Accord.
