We all need to keep evolving our surgical techniques. You would never do a routine couching of a nucleus by pushing it into the vitreous. You would never do a routine intra-capsular cataract surgery with a cryo-probe. You would never use a Von Graefe knife to slice half of the cornea just to access the lens. So please, let's also commit to never using scissors to make the incision for an ECCE surgery! There is still a place for doing great manual extraction of the nucleus, but the incision should be made properly, in the manner of the MSICS procedure. That means a good shelved incision with a long tunnel length created with a crescent blade.
