Today Michael Hoch brings us a structured approach to spine lesions.
This is the third of five unknown cases. This lesion does have aggressive features, namely osseous and cortical destruction with soft tissue extension. Its association with pain in the patient is also somewhat worrisome.
For aggressive lesions, other features you should think about are:
the primary location of the lesion - vertebral body
whether the patient has systemic symptoms – yes (fatigue and malaise)
There is a relatively broad differential in this case which includes
Metastases
Lymphoma
Leukemia
Myeloma
Infection
However, given the patient’s young age and lack of a systemic malignancy, lymphoma or leukemia is favored.
The diagnosis is: spinal lymphoma
