The administration of incorrect medicine, especially if it is administered intravenously and the medicine is a high-risk medicine, can be extremely dangerous (potentially dangerous with the possibility of serious adverse effects on the patient).
While many medication errors do not cause serious harm to patients, some medications are known to carry a higher and more serious risk than others, and any errors in their administration can result in disastrous clinical outcomes. High-risk or high-alert medications are what they're called (HAM).
They're involved in the vast majority of medication errors or sentinel events, and they're linked to a high risk of abuse, error, or other negative consequences.
