This injection is seen with a short axis view of the proximal patellar tendon. The needle is a 25g spinal needle and the approach can be lateral to medial or medial to lateral. The hyperechoic needle is seen deep to the tendon. You can pass the needle directly into a larger area of tendinopathy if desired. You can use 1cc of lidocaine just to anesthetize the skin and immediate subcutaneous tissues which will make the procedure more comfortable. Ice can be used after the procedure.
