Health Call Virtual Ward was developed in late 2020 in collaboration with front line teams at South Tees Hospitals NHS Foundation Trust and Tees Valley Clinical Commissioning Group. It was initially developed as a ‘step down’ service from secondary care, but was developed further so primary care teams could monitor patients remotely. It means that patients who might be at risk of deterioration can be quickly identified and treated.
