One of the most important documents to keep track of while traveling is your passport. So what happens if yours gets misplaced or is stolen while you are abroad? What if this occurs while you're still at home in the United States? See below for details on what to do if your passport is lost or stolen.
How to report and replace a lost or stolen passport while overseas : If you lose U.S.A. passports while living or traveling abroad, you must immediately contact the closest US Embassy or Consulate to report it. You must visit in person and apply for a new passport. Click here for a listing of all US Embassy and Consulate websites. At each website you will be able to find addresses and phone numbers of the embassy or consulate, as well as more information on what to do in the case of lost or stolen passports. How to report a lost or stolen passport while in the US: If it has become lost or was stolen while you were in the United States, you must report this information immediately. You may do so by completing the steps below. Download and print Form DS-64, the Statement Regarding Lost or Stolen Passport. You must have Adobe Acrobat reader to download this form as it is a PDF file. Click here to download Adobe Acrobat Reader. Fill out as much as the form as you can, answering the information questions in detail and sign your name at the bottom. If you would like to report your missing passport, but not apply for a new one, then you must submit Form DS-64 to the following address:

Washington, DC 20036 If you would like to report your missing passport as well as apply for a new one, then you must fill out a new passport form, and bring both forms in person to the nearest Passport Agency or Passport Acceptance Facility. How to replace a lost or stolen passport while in the US: After you have reported your passport lost or stolen, you must replace your lost one with a new one. Here's how to get it. You must download, print, and complete Form DS-11, the Application for Passport, as if applying for a new U.S.A. passport. Click here for more information on this process. When completing Form DS-11, answer question #18 in the following manner: Write your name as it appeared in your passport. Write the approximate date of issue. Include the passport number if known. Under "DISPOSITION", mark the appropriate box. If your passport was expired, write "EXPIRED" next to the "OTHER" box. Bring your completed Forms DS-11 and DS-64 to the closest passport Acceptance Facility or Passport Agency. You will also need to bring along two 2x2 photos, Proof of US Citizenship, and pay the applicable fees. If you recover the passport previously reported to be lost or stolen, it is now invalid. You must immediately submit it to the following address to be destroyed or invalidated and returned to you.
