Form DS-11
Proof of U.S. citizenship (such as a birth or naturalization certificate)
Government-issued photo ID (such as a driver's license or military ID)
Color passport photo.
•Fee paymet (check or money order)
*Obtain your TWIC Card

*Apply for Merchant Mariner document (MMC)

When applying for document also remember to apply for the medical certificate. They are both on the same website.
