My relative complaints of discharge from nipple ! Is this a worrisome sign?

Nipple discharge is the third most common symptom patients presents to us at the OPD. There are various kinds of Nipple discharges. The physiological Nipple discharge is the milk. Other than milk secretion is pathological. Patients can complain of yellow color or straw colour fluid coming out form nipple or discharge from pus. Sometimes this yellow discharge cam be mixed with pus or there can be frank blood discharge also. There are various factors that we need to consider before evaluating this. We need to see that is the kind of discharge, the quantity, whether it is is presents on the single side or both sides of the breast and we need to see any associated features like if there is any mass present in the breast or any skin changes like any ulcer or any discoloration of the skin or any itching over the nipple area. So all these associated factors also need to be looked after . So based on these findings we will be evaluating the Nipple discharge. Most of the times, 80% of the times the Nipple discharge is non cancerous in nature. It can be just a small infection or any benign conditions of the breast. Only 5 to 15% of the times they can be related with cancer condition. So how do you evaluate the Nipple discharge? First the physician evaluates for presence of any lumps in the breast or the axilla . After that there will be investigations. Whatever discharge is coming out of the breast will be examined under the microscope to look for any oncological cells if present. There is a spherical tests called as ductoscopy where a small camera will be out through the nipple area through the ducts to look for any pathology. We can also ask for any investigations like ultrasound or mammogram to look for any lumps or any precancerous or cancerous conditions which could have been missed out clinically in a non palpable case. So based on the findings on all these things further management will be discussed.
