This young woman suffered a fall as an infant and suffered an injury to her left temporomandibular joint (TMJ). This overtime resulted in left-sided ankylosis of the TMJ. This caused deviation of her mandible to the left side and resultant asymmetry. There was difficulty with mouth opening. She presented to our hospital for corrective surgery.
She initially underwent TMJ ankylosis release with a gap arthroplasty with temporalis muscle interpositioning. Her mouth opening improved greatly, but she still had facial asymmetry.
She now returns for correction of her facial asymmetry through unilateral distraction osteogenesis of her mandible. Treatment planning was presented to her and she consented to surgery.
Bone cuts were made in the left mandible and the uni-vector distractor device was fixed to the mandible. A Le Fort I osteotomy was also performed for the correction of her occlusal cant. A total of 19.5 mm of distraction was performed with 1 mm of distraction done every day.
The distractor was left in place until there was complete bony consolidation at the distracted site. The patient was very happy with the results of the surgery. Facial symmetry had been established along with improvement in function following surgery.
