Where to bring Alexander's Innards Key Item in Elden Ring. You bring them to Jar Bairn at Jarburg in Eastern Liurnia. You will need to go through several steps before giving the innards. This quest is a new addition with Patch 1.03

Extra/Important Info:
Alexander's Innards are obtained by finding and defeating Alexander in the Crumbling Farum Azula. This happens after first finding him in Limgrave to free him of a pit he got stuck in, then battling Radahn with him, then speaking to him in the battlefield after the Radahn battle. You then need to find him in the Lava near where the Lava Worm spawns by Hermit's shack. Find him once more near Jarburg and use oil pots to help unstuck him from another pit. Defeat the Fire Giant with his aid (possibly optional) then meet him in the Crumbling Farum Azula for the fight.
