COVID Monitor is being introduced into the West Metro COVID Positive Pathway. COVID Monitor is a secure, online portal that allows you to view all of the patients enrolled in the model from your practice in one, clear dashboard. In addition, COVID monitor has self reporting symptom monitoring for patients that will allow flexibility for follow up of patients. This is a demonstration by the NWMPHN Director of Primary Health Care Improvement, Bianca Bell, and look at COVID monitor and how it will assist the care of COVID positive patients in the community.
