Two sequential CTOs, one in the proximal circumflex and one in the first OM, at the touchdown of a SVG. Used antegrade wire escalation and retrograde crossing to recanalize the first CTO and antegrade wiring to cross the second. Thrombus and dissection occurred at the SVG touchdown treated with aspiration thrombectomy and implantation of an additional stent with an excellent final result.
