Subacute Thyroiditis (De Quervain’s Thyroiditis) – Causes, Symptoms, Diagnosis and Treatment
Subacute Thyroiditis (De Quervain’s Thyroiditis) is an inflammatory thyroid condition often triggered by a viral infection, such as Coxsackie, mumps, or influenza, following an upper respiratory tract infection. It typically presents with gradual onset of moderate to severe thyroid pain radiating to the jaw or ears, fatigue, malaise, low-grade fever, and a tender, enlarged thyroid gland. The condition progresses through three phases: a thyrotoxic phase (hyperthyroidism symptoms due to hormone release from damaged thyroid tissue), a hypothyroid phase (fatigue, weight gain, and cold intolerance as hormone levels drop), and a recovery phase where thyroid function normalizes. Diagnosis is based on clinical history, elevated ESR/CRP, abnormal thyroid function tests, and low radioiodine uptake on scans. Treatment focuses on supportive care, with NSAIDs for mild pain, corticosteroids for severe cases, beta-blockers for hyperthyroid symptoms, and thyroid hormone replacement if hypothyroidism persists.
