Multiple risk factors for BE?
Chronic GERD (eg, over 5 yr)
Male sex
White race
Hiatal hernia
Central obesity
Tobacco use
First-degree relative with BE or esophageal adenocarcinoma

Screening upper GI endoscopy
↓
Changes in distal esophagus:
Columnar epithelium
Intestinal metaplasia (ie, goblet cells)
↓

No dysplasia → PPI, repeat endoscopy

Low-grade dysplasia → PPI; surveillance or endoscopic eradication

High-grade dysplasia → PPI; endoscopic eradication
