n this video, we will present our elaborate description of ‘Contextual Factors’, based on the work in the OMERACT Contextual Factors Working Group. You will learn about the three types of contextual factors that are relevant within clinical trials. We have termed these ‘Effect-Modifying Contextual Factors’ (EM-CFs), ‘Outcome Influencing Contextual Factors’ (OI-CFs), and ‘Measurement Affecting Contextual Factors’ (MA-CFs). Each contextual factor type will be introduced with an example. In the end of the video, a summary of the three types is provided.
In short:
- ‘Effect-Modifying Contextual Factors’ (EM-CFs), modifies the treatment effect (i.e. some patient subgroups experience greater or less effect from a treatment compared to other subgroups), and are relevant for personalized medicine.
- ‘Outcome Influencing Contextual Factors’ (OI-CFs), sometimes called risk factors predicts the natural course of a disease (the prognosis), and they may confound the results of trials which are not randomized.
- ‘Measurement Affecting Contextual Factors’ (MA-CFs) influences the performance of outcome measurement instruments (such as the reliability, validity, responsiveness, etc.).
