Hydatid cyst of liver is a parasitic infection of the liver, by a Tape worm called Echinococcus granulosus.
This is a zoonotic disease, means a disease found in animals like the sheep and cattle and humans are an accidental / intermediate host.
Life cycle of the Tape worm, exhinoccus granulosus:

The definitive host (In whom the adult tape worm lives) is the intestines of the dog and the lay the eggs in the intestine of dogs. The eggs are excreted in the feces by the dogs and contaminate the surroundings. The eggs are ingested by grazing sheep and cattle, in whom the larve form and enter the intestine wall and into the blood stream and lodge in the organs like the liver, spoken, lungs and the brain and bone.
Once lodged in these organs the larva slowly grows to form the Hydatid cyst.
When the dogs again eat the meat of sheep and cattle contaminated by the larvae, the level enter the dog's intestine and then these larvae become the adult works.
How do humans get this disease?

People living in and around sheep and cattle, those who rear them get this disease by accidental ingestion of food contaminated by the tape worm eggs.
What are the symptoms?

The larvae grow very slowly and hence the symptoms are trivial.
- dull aching abdominal pain
- fever with chills
- jaundice
- vomiting
How to diagnose?

- ultrasound abdomen is suggestive , the typical findings may be confused with a simple Liver cyst, but if the membrane and the hydatid sand are seen them itsvery suggestive,
- CECT scan
- MRCP
What are the complications?

- infection and pus formation
- rupture into the bile ducts causing jaundice and cholangitis
- rupture into the peritoneal cavity and dissemination
What is the treatment?
************************
I) Surgery is the only treatment.
A) Cyst pericystectomy: The cyst and its surrounding wall have to be removed completely, the cyst is sterliEs before handling by injecting scolicidal agents to kill the live elements of the cyst.
B)Resectional surgery , or removal of the part of the organ involved is the most radical treatment.
2) Non surgical treatment - in selected cases when it's a solitary cyst without biliary communication, then PAIR treatment can be done. PAIR stands for Per cutaneous aspiration injection and reaspiration. Here the cyst is aspirated under ultrasound guidance, injected with scolicidal agents and then reaspirates . Here the cyst is killed without surgery.
3) Medical treatment: Anti helminthic drug like Albendazole is given for small and deeply located cysts and as a part of surgical treatment given before and after the surgery.
A dead and calcifies cyst does not need any treatment.
