Screen Pop for SugarCRM looks up leads, accounts and contacts based on either incoming or out going phone numbers.
Screen Pop integrates with Asterisk and all that is needed to use it is to pick up your ip phone and dial a number or, answer an incoming call.

Screen Pop also works with Click to Call features.
