Today Michael Hoch brings us a structured approach to spine lesions.
This is the fourth of five unknown cases. This lesion does have aggressive features, such as soft tissue extension, cortical destruction, and irregular margins. In fact, the lesion is extremely aggressive looking
In this aggressive lesion, other features you should think about are:
the primary location of the lesion – posterior elements
whether there is matrix formation – yes (osteoid)
In an aggressive spine lesion with osteoid matrix, there is a very limited differential, of which the main consideration is osteosarcoma.
The diagnosis is: spinal osteosarcoma
