Gameplay Part 19 of The Witcher 3: Wild Hunt...

The finale! First Geralt goes to Novigrad to meet with Dijkstra, Roche and Thaler. Joining the conversation, Dijkstra will reveal his plan on how to lure Radovid out using Philippa Eilhart as bait so they can assassinate him. Outside, Philippa, says their plan would never work, as Radovid may be insane, but not stupid and wouldn't fall for empty words. She then gives the witcher Vizimir II's ring, to use as proof to get Radovid to believe Geralt and maybe even trust him completely. However, he doesn't believe you and Radovid orders his men to secure the bridge so that no one gets in or out, but then also orders Geralt killed while he is without his swords. Fortunately, Roche's ambush saves him and Geralt must now join the fight to defeat the Redanian soldiers and witch hunters on the bridge. But aI had to give up on this quest, effecting the outcome, making Radovid king and killing all witches, sorceress and non-humans in the cities.

Back to the main quest "On Thin Ice". Avallac'h will Sunstone to summon the Hunt, while the Lodge prevents them from retreating with their magic and Emperor Emhyr's fleet stops the ship physically. From there, Geralt needs to attack the ship and fight Caranthir and Eredin. Ciri is not happy about waiting on the sidelines. Avallac’h can tell that she cannot be stopped. Ciri teleports onto the frozen water and enters a rage in front of the Hunt. She fights her way to Caranthir. Eventually, Ciri destroys Caranthir’s staff. This breaks the ice curse that has frozen Geralt. Ciri is knocked out by this powerful blast, but Geralt walks down to finish off Caranthir, also dodging Ice Elementals he summons. With him almost dead, he teleports the two of them beneath the ice and into the water. Geralt swims up to the shipwrecks and war going on. He makes his way to Eredin on the Hunt's ship. A very long intense fight ensues (after multiple attempts), Geralt defeats him and is saved from a hoard of Wild Hunt hounds by Yennefer.
The ending "Tedd Deireadh, The Final Age" is depended on choices you've made in the game. Avallac’h has taken Ciri and is using her to cause the worlds to collide. Yen and Geralt use a portal to appear in this space and navigate up to the tower they are on. Ending #1, the most positive ending:
After the events on Undvik, Geralt goes to see Emhyr var Emreis and tell him that his daughter sacrificed herself to prevent the White Frost from consuming the land.
Afterward, he makes his way back to the former garrison outpost in White Orchard, where he receives a specially-crafted silver sword from a hired blacksmith. He then returns to the White Orchard Inn, where he meets Ciri in disguise and presents her with her new Witcher sword - a tool she is eager to test in the field.
