This patient is a 79-year-old female who presented with a substernal chest pain, which has been a chronic issue for the last 20 years. She also complained of occasional reflux and upper GI demonstrated a very large hiatal hernia. In addition, the endoscopy demonstrated esophagitis and potentially a short esophagus. Dr. Nguyen and his team will have to see intraoperatively whether they can reduce the entire hernia and perform an anti-reflux operation. If the patient does have a short esophagus, they will most likely repair the hiatal hernia with mesh and perform the gastropexy.
