Hi, I'm Dr. David Stoker, a board-certified plastic surgeon in Los Angeles. I wanted to tell you about one of my favorite cases of all time on patients who have lost an extreme amount of weight. This patient was a 30-year-old woman who lost over 200 pounds through diet and exercise. She was part of the TV show, Extreme Makeover Weight Loss Edition, that I also was a part of. She really had an inspirational progression over the course of a year through intense diet and exercise, working out more than four hours a day, basically doing everything she was supposed to do and then some, and had tremendous success.

Unfortunately, her body didn't just snap back after gaining and losing over 200 pounds. Her breasts were very droopy. Her arms were very droopy. She's only 30-years-old, and much looked much older than that. Her abdomen was a mess, both from the excess skin, pubic laxity, and thigh laxity, but also from previous surgery. I think it was an appendectomy. We did a bilateral arm lift on her. We did a breast lift, but also we extended that laterally for what I call a lateral chest skin excision. You can see that on the oblique views. Then I did a big extended abdominoplasty and lateral thigh lift. That means I just kept going almost 360 degrees around her in a pubic lift because there was so much laxity of her mons pubis; it needed to be addressed separately. I did that all at one time. She healed beautifully, with no problems.

What was most exciting for me about this is what an impact it had on her life. If you'll notice, she's tan in her after pictures. The reason is that she's been wearing a bikini for the first time ever in her adult life. She was pretty well-known and celebrated for her success in weight loss and being cute and just a great personality and just a nice person overall, and actually Good Morning America, the TV show, invited her on to show off her results in this transformation. It was kind of a fun conclusion to a really dramatic story. It captures a lot of the essence of what I love most about plastic surgery, being able to make a really impactful change in people's lives to help them in a significant way all of a sudden in one day.
