An abscess s is nothing but a collection of pus. Pus is nothing but when the tissue gets infected, there will be a fight between the bacteria and the white blood cells to conman that. So in the process the bacteria and the white blood cells, some of them die and that is called as pus. That pus has to come out. That is how the treatment of abscess in the body. Basic principle is you have to drain it. So perianal abscess is a collection of pus around anal region. Commonest sources of infected is the anal glands which is situated between the two sphincters which controls the continence mechanism of the anal canal. So once the pus forms there, it can come between the sphincters and drain or it can be piercing the sphincters and spreading there what we call as ischorectal abscess, supralevator abscess, depending on the site and the size, we decide how to operate on that. After surgery, why do some of them recur? The basic problem with recurrence is one is inadequate surgery. If it is not rained properly and if there is some collection, then it can recur, or if these once is not clear, the chance of recurrence is quite high. So a simple drainage of perianal abscess , the recurrence rate or the c chances of it becoming a fistula is as high as 40 to 50% or eve more, whereas a properly drained abscess can recur in a certain percentage because of the reinfection of the glands nearby. For the reason which the patient who already had it. So to prevent recurrence, one is at the first surgery it should be cleared properly and the chances of recurrence are very low and after the procedure, keep the cavity opened, so post operative care is very important in preventing the abscess and any case for abscess like chronic constipation or too much of friction in that area by cycling , motor biking or too much of travelling can cause recurrence of perianal abscess.
