The visible part of the inferior vena cava through subxiphoid echo window is completely packed with extended part of the Wilm's tumor into right atrium. The tumor was visible in the pulmonary artery .The right ventricular systolic pressure was normal. The use of preoperative chemotherapy in the initial management of advanced Wilms's tumor extending into the right atrium is quite useful and must be attempted whenever possible
