Miguel Borja scored a penalty in the extra time of the second half, making the Millonarios beat their historic rival at home.
Minutes later, a fight broke out on the pitch, when River's Agustín Palavecino celebrated the goal to Boca's Nicolas Figal and goalkeeper Sergio Romero reacted.
Coaching staff and players on the bench entered the pitch, some to fight, some to separate them.
Six players were expelled after that: Boca's Nicolás Valentini, Miguel Merentiel, Ezequiel Fernández and head coach Jorge Almirón; and from River, Palavecino and substitutes Elías Gómez and Ezequiel Centurión.
