In the case presented here, the patient has a long-standing, dense, white cataract that has caused some scarring to the posterior capsule. After successfully removing the cataract, you notice that the posterior capsule is scarred. Your best attempts at irrigation/aspiration and capsule vacuuming have not helped. Even your diamond dusted polisher is of no help. What do you do now? Should you leave it as is and then do a YAG laser capsulotomy a month or two later? Or should you attempt a primary posterior capsulorhexis now?
