58 year old female with left hydronephrosis and flank pain despite ureteral stenting. Diagnosed with retroperitoneal fibrosis following negative percutaneous biopsy. Persistent retroperitoneal mass and ureteral obstruction following trial of steroids. Underwent ureterolysis and omental wrap with stent removed after 4 weeks. Hydronephrosis and pain resolved.
