Ruptured disc at L4-L5 affecting the L5nerve root. Disc herniation at L3-L4
•Motor L4: ankle dorsiflexion (tibialis anterior). Reflexes: patellar reflex (the patellar reflex is mainly L4)
Positive femoral stretch test: the test is positive if the pain is felt in the ipsilateral anterior thigh.
Disc herniation at L4-L5
•Clinical evaluation of L5 nerve root: sensory: L5 dorsum of foot and leg. L5 extension of the toe.
Positive straight leg raise test= tension sign
Very important test to determine whether the patient with low back pain has underlying herniated disc.
Disc herniation at L5-S1
•Clinical evaluation of S1 nerve root: Sensory: S1 lateral and plantar aspects of the foot. Motor: S1 foot plantar flexion (gastro-soleus). Reflexes: S1: ankle reflex.
