State policymakers are increasingly asking Medicaid managed care plans to address social determinants of health (SDOH) and improve health care outcomes. The Center for Health Care Strategies (CHCS), in partnership with the Association for Community Affiliated Plans (ACAP), recently published a report reviewing the tools and strategies that policymakers are using to build stronger linkages, referrals and coordination between plans and social service providers—including schools. This webinar provided an overview of the types of arrangements that encourage Medicaid health plans to partner with other sectors (including education) to address SDOH, including care coordination, quality assessments, and value-added services. This session was intended to provide Healthy Students, Promising Futures Learning Collaborative members with a better understanding of how states and plans work together to address SDOH, and how their state team can engage.
