Turns out 4 AWG connectors are too large for 4 AWG stranded THHN ground wire. Apparently 4 AWG wire with coarse strands have a smaller diameter than 4 AWG wire with finer strands. The solution is to use 6 AWG connectors despite the cable being 4 AWG. Warning: measure the diameter of your wire and connectors, and validate that the connectors you are using are the appropriate rating for the work you are doing.
