This video show video endoscopy of larynx in old Man .
He presented with complain of change in voice , Post nasal drip & cough during drinking water since last 2 months . No history of trauma , surgery , difficulty in breathing & swallowing .
Endoscopy of Larynx show Left vocal cord in Para-median position .
Unilateral injury or compression to recurrent laryngeal nerve results in paralysis of all the intrinsic muscles on that side except the crico-thyroid. The vocal cord is median or para-median position and does not move laterally on deep inspiration .
Unilateral recurrent laryngeal paralysis may pass undetected as about one-third of the patients are asymptomatic. Others have some change in voice but no problems of aspiration or airways obstruction. The voice in unilateral paralysis gradually improves due to compensation by the healthy cord which crosses the midline to meet the paralysed one.
X-ray Chest , CT or MRI Head , Neck & thorax is advised to known the cause for Vocal Cord Paralysis .
In this patient , CT Scan show multiple Mediastinal Hilar Lymph nodes with minimum bilateral pleural effusion . Patient is referred to Pulmonologist for further evaluation .
