2017 Florence Schorske Wald Lectureship on Palliative Care & Hospice Care, sponsored by the School of Nursing, K.J. Densford Center as a pre-conference session to the Minnesota Network of Hospice & Palliative Care (MNHPC) annual conference. Recorded April 2, 2017 at the Doubletree Hotel, Bloomington, MN.
Jeff Cohn, MD, MHCM, the keynote speaker spoke on the topic of “Making the Difficult Conversation Simple(r)”. This half-day workshop focused on the role of conversation leading to advance care planning and person-centered care for individuals with advancing serious illness.
Attendees learned about:
• Advancing serious illness
• Knowledge/communication gaps being faced by patients with advancing serious illness, and how they vary by underlying conditions
• Creating conditions for psychological safety, enabling communication about sensitive information comfortably and safely
• An experience with an evidence-based communication tool, My Gift of Grace
• Interactive activities that lead to participants leaving the workshop with personal commitments to action regarding enhanced conversation with their patients dealing with advancing serious illness
