Penalties and threats are not effective policies. First, it is important that you understand the goal of your policy. You don't care about the cancellation! You care about the decline in revenue and realized expense of overhead. If you can find a way to increase revenue and optimize overhead you wouldn't be just fine with a patient who cancels or no-shows.
Don't penalize your patient for a poor business model. Instead, figure out how to make a system that achieves better patient outcomes, increases profitability for the practice, and fosters creativity in your clinical staff.
