While possibly the most difficult trophy in the game, it is significantly easier than many realize. It does not mean you need to hit every single note, you just need at bare minimum, a 90% score (SS). You can make 1-2 mistakes on the song and still hit SS as long as you were hitting your notes correctly in the 105-115 range. To get that 105-115 range, you need to make full swings by using your wrists more than your arms and try to aim in the middle of the note to maximize the score.
