Robrecht Knapen, MD, Maastricht University Medical Center, Maastricht University, Maastricht, Netherlands, discusses results from a sub-study of the MR CLEAN-LATE trial, which examined first-line thrombectomy techniques and outcomes in late-window stroke patients. The randomized, controlled trial included patients within 6-to-24 hours of stroke onset and compared best medical management alone to best medical management with endovascular thrombectomy. Subgroups of patients received stent retrieval alone, direct aspiration alone, or a combination of techniques. No differences in clinical outcomes were found, indicating results similar to those in early-window clinical trials. However, a difference in safety outcomes was observed with direct aspiration, which resulted in higher rates of intracranial hemorrhage. This finding conflicts with previous knowledge that stent retrieval damages vessel walls and was thought to cause a higher risk of hemorrhage.
