Gynecomastia in bodybuilders is the second-most common cause of gynecomastia in men behind pubertal gyno and is the often the result of using steroids or prohormones. At its base, both forms of gyno develop from excess estrogen which causes excess breast tissue, or gyno tissue, to develop. However, gynecomastia in bodybuilders is typically much more dense, more likely to be inflamed and is accompanied by next-to-no fatty tissue than other cases. Our patient today at the Austin Gynecomastia Center is one such case who sought out expert gynecomastia surgeon Dr. Caridi to treat this often mishandled and more difficult to treat type of gyno.
The appearance of gynecomastia in bodybuilders is often displayed as puffy nipples that give an odd shape to the chest, with the nipple-areola complex looking downwards and an area of fullness along the bottom of the muscle. Because these men typically have little body fat, the gyno is mostly a firm, rubbery gland that is overtly noticeable and easily felt upon examination. These patients tend to also have a deceivingly large amount of gyno tissue that is not evident until it is being removed. Suffice to say, for men who pursue to perfect their bodies and definition, this is extraordinarily disturbing to them. Compared to pubertal gynecomastia, pain around the glandular tissue is more common for bodybuilders with gyno. This pain is an indication of inflammation and irritation in the adjacent nerves, which can be quite painful and can be set off by touch or rubbing on the area.

The surgical procedure for gynecomastia treatment in bodybuilders is different in many ways to traditional gyno surgery, however, all the gyno tissue must still be removed to avoid the chance of recurrence, or regrowth. It is a much more difficult procedure with a smaller margin of error due to the small amounts of fat present, is a tougher than normal tissue which can be adhered directly to skin and muscle, and due to the inflamed tissue which has higher levels of blood present, is a much bloodier removal. While liposuction is still involved, it only has sparing utility due to the density of the gyno tissue and less prevalent amount of fatty tissue. Overall, the ideal gyno surgery scenario for bodybuilders is to get rid of all the gynecomastia tissue, not leave a depression in the skin, allow the pectoral muscle to show, precision contouring of the chest, minimize complications and avoid any hematomas or seromas.

After a highly successful, yet challenging procedure, Dr. C discusses the near immediate results of the patient at their 24-hour post-op appointment. The patient is exhibiting a little bit of swelling and bruising that is typical at this very early stage of recovery, but it is abundantly clear the procedure was a success. There is a nice and clear outline of the patient’s chest, the lateral borders of his pectoral muscles are defined and clean. The sheer amount of gynecomastia tissue removed compared to what one would expect before the surgery is characteristic of gynecomastia in bodybuilders, which is analogous to the appearance of a glacier.
