Lasers still play a role in revising certain types of acne scars in suitable skin types, however most leading dermatologists and plastics would agree that the majority of patients can benefit from no downtime or low downtime procedures such as dermal fillers – Bellafill in the US or Juvederm Volbella in Australia. New insulated microneedling energy devices such as Infini, combined with PRP or platelet rich plasma has really changed the landscape for acne scar revision. My Infini Plus PRP ‘over and under technique’ will be published in 2017- videos with many before and afters to come. I find this treatment far superior to any laser – including the most powerful Co2 lasers. Though my field is in laser dermatology, I find that I am using lasers less over the past one year – exceptions are shallow box car scars, open pores in light skin types 1-2 when fully ablative hybrid laser is still Gold Standard. One treatment and up to 90% of these scars can be treated.

Here is my tip on finding the best specialist to treat your acne scars- go to realself.com and look at the acne scarring area. Find a specialist that is trained in all methods for treating the scar type- this includes the following.

Ice pick scars are best treated with chemical peels such as TCA CROSS or punch excision
Rolling scars are best treated with fractional lasers Fraxel, INFINI Microneedling, Dermarollers, skin needling and subscision
Box car scars are best treated with laser resurfacing using a co2 or erbium laser such as Profractional
Atrophic scars are best treated with fillers such as Juvederm, Restylane, Bellafill, or even fat transfer

Most clinics will by default ‘prescribe’ a laser treatment such as Fraxel, fractional laser, or have an answer such as Derma Roller or microneedling. Whilst these treatments can give some results, they do not offer Gold Standard acne scar treatment solutions. Some Specialist laser and subscise, but do not know how to utilized different filling methods. A bespoke or individualised treatment approach is best, because it is your scars that are being treated and not just a device that is being used for ‘acne scarring.’ If you look at my acne scar before and afters you will notice one trend- everyone will have a mixture of scars and very rarely will a person have only one scar type. Most patients will present with a variety of scars, some patients will have more atrophic acne scarring, whilst others will have deep ice pick scars. So the very first step I do is to ask- what types of acne scars do you have? What gives me the most information is angled lighting. This is how you visualise scars, and it is the same method I use to find acne scars, map them out, and then treat them. Stretching and touch is vital as this tells me if scars are atrophic or anchored. Animation is also vitally important. Sending a picture tells me some information, but the 3D lighting, touch and muscle movement tells me the other half of the picture. Hence I apologise in advance for not providing online advice. It is impossible to give a plan based on static photos- this can be misleading. Very often I will only know how much ‘give’ is in the scar when the needle goes in.

Any specialist who does scar revision to a high level would use angled lighting to determine scar types and if appropriate ask you to animate. Shining direct and bright surgical lighting reveals very little, but by dimming lights and shining light at different angles, touch and movement reveals a whole new world of acne scarring. In many of my upcoming videos you will see me using newer techniques such as INFINI PRP with PRP mask occlusion, and various subsciion filler combination techniques.
