It is the sixth track of the second disc of the Clannad Original Soundtrack album. It was released on August 13, 2004.

It features prominently in many of the series' most powerful moments, especially in ~After Story~; for instance, it is played when Tomoya proposes to Nagisa in "Sudden Events".

A longer and more orchestral version of it, "The Place Where Wishes Come True II", is the fourteenth track of the second disc of the Clannad Original Soundtrack album.

"The Place Where Wishes Come True" was covered in metal style as the fifth track of the album "Keeper of the Metal Key Part II" by the doujin metal band "South of Heaven".
