Course Description
The Use of Post-Acute Care Coordination in Reducing Readmissions in High-Risk Populations will define what a "readmission" is and the importance of reducing readmissions. Knowing this will help attendees understand the impact of readmissions on the healthcare system. This program will also review how physiatrists (PM&R physicians) have helped reduce readmissions in Skilled Nursing Facilities and the results of two specific studies. In addition, the program will examine the Transitions of Care (TOC) Programs and their results. We developed this program to help inspire healthcare providers to think outside the box to reduce readmissions across the continuum of care.
Course Relevance
This program is relevant for NURSING(Advanced Practice Nurse(APN), Registered Nurse(RN), Licensed Practical Nurse (LPN)), SOCIAL WORK (Licensed Social Worker (LSW), Clinical Licensed Social Worker (CLSW), Licensed Medical Social Worker (LMSW), and Licensed Clinical Professional Counselor (LCPC)), and LONG-TERM CARE ADMINISTRATORS (Nursing Home Administrator (NHA), Resident Care/Assisted Living Administrators (RCAL), and Home and Community Based Service Executives (HCBS)). It will help participants understand the direct impact readmissions can have, not only on their facilities but also for their community partners. In addition, it will discuss the importance of reducing readmissions across the entire healthcare system. This program will help participants think outside of the box. Data on how a physiatrist may help reduce readmissions, as well as reviewing the Transitions of Care (TOC) Programs and their results, will help them think outside the box when creating a new TOC Program or improving on an existing one.
