My gear is philosophy is, in order of importance is:

+Base damage / +%Attack damage
+%Crit Damage
+Attack Speed
+Movement Speed
+Passive Dodge Chance
+%Physical Damage or Shadow Damage or Elemental Damage
+Crit Chance
+HP / HP Regeneration

As much HP as possible on Chest and Helmet

I use 2 copies of mark of impurity. One for groups of enemies, and one for single target. It's not necessary but it saves a little bit of time. I try not to use Havoc Orb too much, but I throw it once in a while on huge groups or big bosses.
